Vitality Hospice & Palliative Care is seeking an exceptional Per Diem Registered Nurse to join our team. Schedule: Per Diem — Every Other Saturday & Sunday

Responsibilities

As a Hospice Visit Nurse, you will:

Provide professional nursing care, including admissions, initial comprehensive assessments, and ongoing evaluations.
Develop, implement, and update individualized plans of care based on patient needs.
Collaborate closely with the hospice physician and full Interdisciplinary Team to ensure appropriate interventions.
Oversee and support the work of home health aides, social workers, and chaplains.
Educate patients and caregivers to promote safety, comfort, and understanding.
Maintain high standards of documentation, compliance, and clinical excellence.
Contribute to continuous improvement efforts to elevate the quality of care across the agency.

Qualifications

Active NJ Registered Nurse (RN)license
Minimum 1 year of RN experience
Willingness to travel within assigned service areas
Valid driver's license, reliable transportation, and active auto insurance
Strong communication and interpersonal skills
