We are seeking a Substance Use Disorder (SUD) Healthcare Navigator to join our New Day Recovery Program in Bristol, VA , supporting individuals navigating recovery from substance use and co-occurring mental health conditions.
This role is ideal for someone with a background in community health, case management, behavioral health, or peer recovery support who is passionate about improving access to care and addressing social determinants of health.
The Healthcare Navigator plays a critical role in guiding patients through the continuum of care by coordinating services, removing barriers, and connecting individuals to both internal SVCHS programs and external community resources.
This position works collaboratively within an interdisciplinary d requires strong relationship-building skills, patient advocacy, and knowledge of recovery-oriented systems of care.
